Decision Support is an innovative solution that provides actionable recommendations on the type of home and community-based services that will help members achieve optimal independence in their lives. To develop these recommendations, CareBridge's LTSS Specialists, each of whom have a background in physical or occupational therapy, complete a person-centered assessment of each individual, informed by insights from the nation’s largest HCBS analytical database, to identify potential enhancements to the individual’s care plan and recommend empowering alternatives that can keep the member safe and independent at home.
